Medication Formularies
The first formulary below is for Title XIX (19) and Title XXI (21) service recipients. The second formulary below is for Non-Title XIX (19) service recipients with serious mental illness (SMI) and is a generic-only medication benefit.

Crisis Formulary for Non-titled, Non-SMI recipients presenting at an in-network Level 1 Subacute Facility in crisis. Medications are limited to a 10 day supply with no refills.
